West Bend Care Center, our 45-bed skilled nursing facility located in West Bend, IA, offers its residents a home-like environment with the ability to surround themselves with the comforts of home. Prairie Creek Assisted Living is our 16-unit Assisted Living Apartment home. Our residents maintain an active lifestyle participating in daily activities that keep their physical and mental health in mind. Our caring team of professionals works in tandem with our residents' healthcare providers to develop a long-term care plan to maintain quality of life in an environment of safety, trust and respect. Participation in an array of daily activities is encouraged to enhance the physical and emotional well-being of every resident.
